At the end of the assessment process, we will provide you with a detailed report of our findings and recommendations. What is a Supported Living Assessment?

A Supported Living Assessment (SLA) is an evaluation occupational therapists (OTs) provide to determine an individual’s need for support in activities of daily living. This assessment looks at the individual’s current level of functioning and their ability to perform activities independently. The goal of the SLA is to identify what type and how much support the individual may need to live as independently as possible.
Why would I need a Supported Living Assessment?
There are many reasons why an individual may need an SLA. Some common reasons include:

How is a Supported Living Assessment conducted?
